Elderly and adults with disabilities have an alternative to institutionalization at CCS - Yuma's Daybreak Adult Day Health Care, a welcoming and structured day program with nursing services. Daybreak offers interactive and social activities to encourage cognitive stimulation/maintenance and social development. Exercises and nutritional services promote healthy aging. Daybreak provides community-based care, so that a loved one can remain in the home. Family caregivers receive respite time so that they can better care for themselves.

An enrollment packet will need to be completed by your loved one’s physician. You may also speak with a case manager with the Area Agency on Aging or Arizona Long Term Care System (AHCCCS).
